The format guides you step by step from identifying the issue through to planning and tracking actions, ensuring that solutions are practical, measurable, and sustainable.
Instructions
Define the Problem – Clearly state the issue you are addressing.

Understand the Current State – Summarise the situation as it stands now.
Identify the Root Cause – Use tools like 5 Whys or Fishbone Diagrams to dig deeper.
